Can anybody identify this large flare gun?

It is a No.4 MK 1 *, in 37mm, manufactured by the Molins Machine Co. of London. It is missing the safety button.

It appears this one has had the short barrel with lugs replaced with a long barrel.

This could have been done by the factory, or by an amateur gunsmith in his garage.

There are a number of different Webley style flare pistols with factory long barrels added. Just guessing, but could it be used as a line thrower ?

Another possibility is that it could be a Middle East/Asian copy made with long barrel, but I doubt that.

The SM prefix to the serial number shows that it was made by Singer Manufacturing Co Ltd, which was part of Singer Sewing company in Scotland.

It has had a barrel extension added at some point, this is not how it left the factory.
